Children's Hospital of Wiscons-Fox Valley will receive a $5,000 donation as Favre's designated charity for the award.<p>
Favre completed 30 of 43 passes for 365 yards and three touchdowns for a passer rating of 109.2 in the Packers' 34-31 win in the Metrodome, which clinched the team's third consecutive NFC North title.
Favre threw touchdown passes to Bubba Franks (22 yards), Javon Walker (9 yards) and Donald Driver (3 yards), then marched the team 76 yards in the final 1:35 to set up the game-winning field goal.
Favre beat out fellow QBs Jake Delhomme of Carolina and Peyton Manning of Indianapolis for the games of Dec. 24-27.
